Breath. — The Neurology of the One Tool You Already Have

Picture a moment you know well.
The email that lands wrong. The sound that is suddenly too much. The transition you didn’t see coming. The moment in the grocery store when the lights and the noise and the smell of the bread aisle all arrive at the same time and something in your chest says: we are leaving now.
In that moment, before you have had a single conscious thought about what to do, your brainstem is already running a program it has been running since before you were born.
It is called the pre-Bötzinger complex. A small cluster of neurons in the brainstem, discovered in 1991 by neuroscientist Jack Feldman and colleagues, that acts as the respiratory pacemaker — the biological clock that generates every single breath you have ever taken. You have never had to remind it. It does not ask permission. It simply runs, endlessly, keeping the rhythm.
Here is the part that matters.
Of all the systems that run automatically in your body — your heart rate, your digestion, your immune response, the dilation of your pupils, the release of cortisol when a threat is detected — breathing is the only one you can also control consciously. You cannot decide to slow your heart down through sheer intention. You cannot will your stomach to digest faster. But you can take a breath right now and make it exactly as slow as you choose.
This is not a small thing. This is a door.
The Nerve That Runs the Show
Running from your brainstem all the way down to your gut is the vagus nerve — the longest cranial nerve in the body, and the primary communication highway of what Stephen Porges called the polyvagal system. It carries information in both directions: from the brain to the body, and from the body back to the brain. Roughly eighty percent of its fibers travel upward. Your body is talking to your brain far more than your brain is talking to your body.
When you inhale, your heart speeds up slightly. The diaphragm descends, the chest cavity expands, pressure changes, and the heart responds — beating a little faster to match the increased volume. When you exhale, the reverse happens. The diaphragm rises, pressure reduces, and the vagus nerve sends a signal to the heart to slow down.
This slight variation in heart rate between inhale and exhale is called respiratory sinus arrhythmia, and it is considered a marker of healthy vagal tone. The more pronounced the variation, the more flexible and responsive the nervous system. The more rigid the heart rate — barely changing between inhale and exhale — the more the system has been under sustained stress.
What this means practically: every exhale activates the parasympathetic pathway. Every single one. The inhale is a mild sympathetic activation. The exhale is the recovery. If your inhales and exhales are the same length, you are essentially breaking even. If your exhales are longer than your inhales, you are accumulating calm.
The Diaphragm Is a Bridge
Your diaphragm is not just a breathing muscle. It is a neurological bridge. It contains mechanoreceptors — stretch-sensitive nerve endings — that fire signals directly into the vagus nerve with every breath cycle. When you breathe slowly and deeply, these receptors send a long, steady signal. When you breathe fast and shallow, the signal is short and fragmented.
Think of the diaphragm as the translator between the part of your nervous system you can speak to and the part that runs the show without asking. Most of the time, those two parts are not on speaking terms. Slow, deep, extended breathing is how they make contact.
For neurodivergent nervous systems that spend more time in sympathetic activation — the state of scanning for threat, filtering the world intensely, processing more than the environment was designed to generate — this bridge matters more, not less. The window of tolerance is narrower. The recovery time is longer. The thing that shortens that recovery time is the same thing it is for every human nervous system. It is the exhale.
What the Research Actually Shows
In 2023, researcher Melis Yilmaz Balban and colleagues at Stanford published a study in Cell Reports Medicine comparing three different breathwork techniques against mindfulness meditation for acute stress reduction. The techniques included box breathing, cyclic hyperventilation with retention, and cyclic sighing — a double inhale through the nose followed by a long, slow exhale through the mouth.
Cyclic sighing produced the fastest and most sustained reduction in physiological stress markers of any technique tested, including mindfulness meditation. Five minutes a day. Double inhale through the nose — a short inhale, then a second short inhale to fully inflate the lungs — followed by a long, slow exhale through the mouth. That was it.
The mechanism: the double inhale pops open collapsed alveoli — the tiny air sacs in the lungs — maximizing surface area for gas exchange. The long exhale then dumps the accumulated carbon dioxide that builds up during stress. It is worth noting that the urge to breathe is not driven by low oxygen. It is driven by rising carbon dioxide. Stress breathing — fast, shallow, upper-chest — does not clear CO2 efficiently. The long exhale does.
Why We Call It Breath.
